Rec Plus provides school-age childcare for children in grades K-6, at neighborhood recreation centers. Children must have completed Kindergarten for summer. Children must be 12 and under.

IMMUNIZATION
Minnesota law requires children enrolled in child care to be immunized against certain diseases or file a legal medical or conscientious exemption. I agree to electronically turn these in for my child to: Recplus@minneapolisparks.org within 30 days of my contract being accepted each season (school year or summer). Records must be received prior to the start date of care.

EMERGENCY TREATMENT
I understand that no emergency treatment may be given without informed parental consent except in a life- threatening situation.
I agree to keep on file at the Minneapolis Park and Recreation Board (MRPB), Recreation Plus Childcare Program current telephone numbers where a parent or designated responsible adult can be promptly reached in case of emergency. In case of emergency while my child is in attendance at the MPRB Recreation Plus Childcare Program, I understand that the following procedure will be followed.
Staff is trained in first aid procedures. If a serious injury should occur, the Recreation Plus staff will perform first aid. Staff will notify you to pick up your child immediately and let you determine if you should take your child to the doctor or dentist. When necessary, 911 will be contacted. Staff will immediately make every effort to notify the parent or persons on the emergency contact card if 911 is called. MPRB accident report forms will be used for all Rec Plus injuries. The parent or guardian is responsible for all medical charges. Medical insurance coverage for the children is the responsibility of the family.
MPRB LIABILITY
In consideration of my child's participation in this activity, I hereby release and discharge Minneapolis Park and Recreation Board (MPRB), and its representatives, successors, and assigns, from any and all liability arising from accident, injury, and illness that they may suffer as a result of their participation in this activity. I (we) also will follow the rules and regulations set by MPRB and above named parties. Parent or guardian must sign for anyone age 18 and under.
BEHAVIOR
In order to ensure the health and safety of my child in attendance at Rec Plus, I/we understand that certain rules must apply to all children in the Rec Plus program. The rules are as follows:
1. Keep hands, feet and objects to yourself.
2. Follow directions and be respectful of the person(s) in charge.
3. Use appropriate language.
4. Respect other people and property.
I/we understand that if I /or my child/ or a person I have authorized to communicate with staff on my child's behalf, display any of the following behaviors it may result in my child being suspended or expelled from the Rec Plus program:
1. A behavior that takes away any person’s right to be safe.
2. A behavior that keeps any staff person from fulfilling their job requirements to be available for all children because of continuous interference of a child.
3. A behavior that includes inappropriate touching of a person’s body, and/or using inappropriate language or actions.
4. Any behavior involving purposeful destruction or theft of property.
5. Blatant disrespect or refusal to follow directions of those in charge.
6. Running away off Park Board grounds from any Rec Plus program location.
If a violation of the rules occurs, the Childcare Specialist and Child Development Manager/Supervisor will work with the parent, child and other staff to develop a plan that addresses the behavior. Each incident will be decided on a case-by-case basis. If I have concerns with another child in the program, I as a parent/guardian, will talk to the staff. I will not approach or address the issue with a child that is not mine. If I am called due to my child’s behavior I must pick up my child within 30 minutes.

MN GOVERNMENT DATA PRACTICES ACT
The Minnesota Government Data Practices Act requires you to be informed that the information which you are being asked to provide on Recreation Plus registration forms is considered private data. This information is useful and important for us to be able to serve your child. Certain information, such as emergency and health information is required by the Minnesota Department of Human Services for programs to have on file in order to serve a child. Other information is desirable for our program to best serve your child but may not be legally required.
The Minneapolis Park and Recreation Board welcomes persons with disabilities to participate in all programs. All information provided to us concerning a person’s disability will be kept in strict confidence.
If you fail to provide required emergency and/or health information, we will not be able to serve your child. You may refuse to provide other information not required by law, and the Recreation Plus administration will make a determination if your omission of information will prevent us from serving your child.
The information provided will be kept in confidence and made available within our program to appropriate staff that work with the Recreation Plus program. In addition, if your services are subsidized, fully or in part, we are required to allow access to your records by the funding source if requested.
FIELD TRIPS
I understand that my child is required as part of the Rec Plus program to attend field trips using school bus transportation if they are scheduled and attending the day of the trip. It is my responsibility to review field trip information. I understand alternate care options are not provided by Rec Plus during the time of the Rec Plus bus field trip. The only exception to this, is for students returning from summer school at or after 2:00pm.
I understand that my child will attend full group Rec Plus walking field trips if they are scheduled and attending the day of the trip. It is my responsibility to review field trip information. Occasionally, walking trips will be optional and open to student choice.
SUMMER MINIMUM ENROLLMENT REQUIREMENT
Families enrolling in Summer Rec Plus must meet the minimum 25 day requirement. This can be a combination of summer school days and/or non summer school days.
SUMMER AUTO PAY
Authorization is given to the Minneapolis Park and Recreation Board to automatically charge your card 14 days prior to the the week of service to cover Rec Plus payment costs including applicable late fees. Rec Plus is not liable for declined cards; payers are responsible to ensure sufficient funds are available if requesting automatic payment and to provide updated card information when necessary. If written communication is not received after written notice of card decline, late fees will be processed 5 days after payment due date. If split payment is requested, authorization form is required. See parent manual for full payment policies and procedures.
I have read and understand and agree to the terms and selections. I agree that no prior notification will be provided unless the date or amount changed, in which case I will receive notice from MPRB at least 10 days prior to the payment being collected. I understand that this authorization will remain in effect until I cancel it in writing. I agree to notify the Minneapolis Park and Recreation Board in writing of any changes in my account information or termination of this authorization at least 2 weeks prior to the next billing date.
SUMMER 2025 FEE AGREEMENT
I am contracting with the Minneapolis Park and Recreation Board for school age child care. I understand our home address determines the rates paid for the Rec Plus program. Non-Minneapolis resident fees are 50% more for leadership fee. I understand I must give 14 days written notice regarding schedule changes, such as changes in contract time, withdrawal from the program, or withdrawal prior to the start of the program. There will be no refunds due to illness, vacation, inclement weather, withdrawal from program under 14 days or school closing. I understand this is not a drop in or hourly childcare. Rates are daily. Fees are due in advance of service. Contracts are required to be on autopay with a credit card with payment withdrawn 14 days prior to the week of service. No money is refunded upon suspension or termination from Rec Plus. A child suspended from school may not attend Rec Plus and no refunds will be given.

All accounts must be in good standing prior to enrolling for upcoming sessions. Placements will not be held for accounts with past due balances.
Specific registration dates will be posted on our website, www.minneapolisparks.org/recplus.
Registration for the following school year begins in Spring. Registration for current families will be available first followed by registration for new/returning families a week later.
Registration opens at the end of summer. Registrations are accepted as space is available at each non-school day location. Cancellations must be received at least two weeks in advance.
Registration for summer care begins late winter.
Registrations are accepted as space is available; an e-mail confirmation will be sent notifying families whether their registration has been accepted. All contract requests are considered waitlist until accepted.

Once you have added a child to your account you can select to enroll them in the child care provided by your district or organization directly from your dashboard.

Your request for child care is submitted and is pending approval from the district. When the district has approved the request, you will receive an email confirmation of the approval.

Once all steps have been completed, your request for non-school day care will be submitted and is pending approval from the district. When the district has approved the request, you will receive an email confirmation of the approval.
Non-School Days can only be canceled from your account page before they are approved by your district or organization. If you need to cancel care on a Non-School Day after it has been approved, you must contact the site administrator for your district or organization (Childcare Specialist or RecPlus@minneapolisparks.org).
Viewing Registered Non-School Days
Once your request for Non-School Day care is approved, you can view those dates by:
Yes, children do need to be enrolled in before or after school care to register for non-school days.
During Summer Rec Plus, "non-school days" are used for days that sites may potentially be consolidated (i.e. day(s) surrounding July 4th). Children need to be enrolled in summer care to register for this/these day(s).
If your child’s contract is approved for the current school year you are considered a current family.
**Be sure to only have one account created for your child(ren). This saves confusion during registration acceptance (i.e., a child appearing as a new family because they were registered under a different account number the previous year)**
Eligible sibling requests may be submitted during current family registration. They will be considered prior to new client registration as space permits.
If your child’s contract is still pending, there is no current school year contract, or the contract ended before the new enrollment date, you are considered a new family.
Rec Plus does not offer immediate drop-in service.
As space allows, added days may be requested via Eleyo in advance. If space is available, they will be approved and added to your account.
To make a request for drop-in care:
1. Navigate to your program's Eleyo site and click Sign In.The Sign In page is displayed.2. Sign into your account.Your Dashboard is displayed.3. Under "Your Accounts" select the desired Child Care Account.

5. Select your attending child and your contract with the child care program, then click Continue.A calendar view for the program is displayed.
6. Select the day/s you need drop-in care.Some programs may offer AM (before school), PM (after school), or Full Day care. Make sure to select the care appropriate to your needs.7. Click Finish.
Your request for drop-in care is submitted and is pending approval from the district. When the district has approved the request, you will receive an email confirmation of the approval.
Transportation to and from Rec Plus is the responsibility of the parents or guardian. During the school year parents can contact the transportation coordinator at their child’s school to arrange transportation between school and Rec Plus. Bus schedule information and schedule changes are also handled through the individual school transportation coordinators. Please inform Rec Plus staff of all bus arrangements. Rec Plus does not provide direct transportation to/from school or communicate with MPS Transportation to receive this information. In the event a morning bus fails to arrive, it is the responsibility of the parent to come to Rec Plus within 30 minutes of receiving a phone call from Rec Plus and transport the child to school.
Rec Plus requires a two-week notice for any schedule changes or cancellations. Changes can be done through your Eleyo account. Please note that once your invoice is generated, you will no longer be able to make changes to your child’s contract for that month unless you contact your Childcare Specialist.

Your request to change the child care schedule is noted on the contract page and is pending approval from the district. Once the district has approved the change, you will receive an email confirmation and the schedule change will be reflected on the child care contract page.
Schedule change requests may be made to go in effect 14 days ahead. If a period has already been invoiced, submit the request as soon as the system will allow and then contact the Childcare Specialist at your site to make further adjustments.
If you have registered for child care with your district or organization, you can request to withdraw your child from that care from your dashboard. You can choose to withdraw from any date within the contracts schedule.
Note: 14 day notice is required to withdraw a contract.

6. Select the last day of attendance and click Request Withdraw. NOTE: if the program has not yet started, this date may be left blank for the contract to be completely removed.
Your request to withdraw from child care is noted on the contract page and is pending approval from the district. Once the district has approved the withdrawal, you will receive an email confirmation and the schedule change will be reflected on the child care contract page.
Emergency Contacts for a child may be added or updated during the Child Care registration process for a new Season (School Year or Summer). If you need to update Emergency Contacts for a child before or after completing a registration, use these steps:
If you have already created the family member/relationship in Eleyo, click Add Emergency Contact and select them from the list that is presented or click Choose.
If you have not created the family member/relationship, click Create New Emergency Contact. Complete all required fields and click Create Emergency Contact.
NOTE: Emergency Contacts must be added for each child separately.
You can use the Edit pencil icon to update an existing contact person or use the red Remove icon to delete that person.
In Eleyo, any user account listed as a parent of the child will automatically be listed as an Authorized Pickup. To add additional family members/relationships as an authorized pickup for your child in Eleyo, use these steps:
If you have already created the family member/relationship in Eleyo, click the "Person" drop-down and select them from the list that is presented. Enter or update the person's phone number, then click Save Authorized Pickups.
If you have not created the family member/relationship, click the "Person" drop-down and select Other. Enter the person's name, phone number, and optionally add a note to describe the person (such as uncle, grandparent, or friend), then click Save Authorized Pickups.
NOTE: Authorized pickups must be added for each child separately.
You can remove an authorized person at any time by clicking the red Remove icon and clicking Save Authorized Pickups.

Adding and managing your payment methods with Eleyo is fast, easy, and secure!
To add a payment method:
1. Navigate to your program's Eleyo site and click Sign In or Your Account.Your Dashboard is displayed.2. Scroll down to the footer of the page and click Saved Payment Methods. If using a mobile device, use the Menu on the left with the compass icon. The Saved Payment Methods screen is displayed.3. Click Add Saved Payment Method.The Add New Payment Method screen is displayed.
4. Select your desired payment type.5. Complete all required fields.All fields marked with an asterisk are required.6. Optionally, if you have enrolled for courses or care, you can select to use this payment method for automatic payment of those contracts.7. Click Save New Payment Method.The payment method is saved to your account.
Once you have saved a payment method, you can use the "Saved Payment Methods" screen to modify or remove your payment methods.
Clicking Remove will prompt you to confirm that you would like to remove the payment method.
Clicking Modify will allow you to modify which courses or care are billed to that payment method.
Note: If you need to update credit card information or bank routing details, you need to remove the current payment method and enter the information as a new payment method.
Email or call the Rec Plus Childcare Specialist if a child is going to be absent from Rec Plus. It is the parent’s responsibility to contact the Rec Plus staff. This procedure assures your child’s safety. If the Rec Plus staff has not been notified and a child does not arrive, they will start looking for the child by calling parents and others listed on the emergency card. Fees are collected for days that a child is absent, and finder’s fees are applied. Schools do not inform Rec Plus of reported absences.
To view your account history:
